Can a patient be on both Coumadin and Lovenox?

Pharmacist to New Grad Nurses

Tip of the day:

Question:

Can patient be on both Coumadin and Lovenox?

Answer:  Yes

Patients who are admitted for afib or DVT/PE would need prompt anticoagulation.  Coumadin prevents vitamin K production in the liver which is needed to make clotting factors.  The effect of Coumadin is not immediate to achieve an INR of 2-3.  Generally, the goal is to achieve an INR of 2-3 in about 3 to 5 days.  In the meantime, Lovenox may be given to bridge.
